If unforeseeable circumstances arise and you cannot keep your greyhound, you must return them to MGAP'S care and we will re-home them.

As per the contract: If you need to return your dog for ANY reason, you agree to give enough notice for MGAP to find a foster or new home. If this isn't acceptable to you, you will be responsible to pay a minimum of $350 up front for boarding costs. If it takes longer than 2 weeks for us to locate an available foster home, you agree to cover all expenses, including additional boarding fees at a rate of $25 per day. You are responsible for transporting the dog to us at a mutually agreed time and date. You are agreeing that now and in the future you will be a responsible care giver to your dog until a home is found though MGAP. Any animal abuse or neglect will be reported to the proper authorities. Rehoming the dog yourself will result in a possible court date with a $500.00 fine or return of the greyhound to MGAP. We ask dogs to be returned with up to date vaccinations, teeth in good condition, as they were given to you.

Your last vet visit was when and for what reason? Which vet Clinic?

When did he have his last flea treatment and dewormer? What type and dosage?
*
When was he last vaccinated for rabies? And core?
*
What behaviour problems does he have? I.E. Chewing, house training, jumping, pulling on the lead, etc
*
Has he ever shown aggressive behaviour? Explain in detail. We need to know when, where, and with whom this occurred for each incident. Please provide as much information as possible.

Under what circumstances have you heard your dog growl?

Describe his personality? I.E. outgoing, timid, loves people, afraid of new people, jumps when excited, needy, independent, cuddly, assertive, submissive etc

Is he still using the crate? If so when?
*
Where does he sleep at nights?
*
What are his likes and dislikes?
*
What brand of food is he eating and how much does he eat per day and at what times?
*
Where is the dog fed? Is the bowl left down? Does he work for his meals, if so how?
*
What cues if any, does your dog know? Have they attended any obedience training?
*
How does he behave in the car?
*
What is his bathroom schedule?
*
How much food are you providing so we can transition to another brand? We require a minimum of 2 weeks of food.

What items are you returning with the dog?
*
We require all the dogs medical records to be faxed to the MGAP office at fax number 506 855-7855 before we can take possession of the dog. Please provide the name and phone number of your vet clinic(s) below.
